National Shortage of Normal Saline: Maine EMS and the Medical Direction and Practice Board would like to alert you to the growing shortage of Normal Saline. Due to unforeseen interruptions in the production line, the national supply of normal saline is in jeopardy and locally, many Maine hospitals are in short supply of normal saline. Maine EMS has received questions from agencies regarding this shortage. Maine EMS and the MDPB have been following this shortage closely and wanted to share a potential action plan with EMS agencies who encounter this shortage locally.
In the instance in which an EMS service is unable to procure normal saline, agencies may substitute with Lactated Ringers for resuscitation and D5W for medication drips.
All medications in the EMS formulary in Maine delivered in a drip may be provided with either lactated ringers or D5W. D5W is the only fluid other than saline that comes is 250 ml bags. If the shortage affects your local community, hospitals have been asked to supply 250 ml bags of D5W for mixing with medications such as TXA, amiodarone, NOREPInephrine, and EPINEPhrine. Please keep this in mind when opening drug boxes. For your awareness, we have attached a second memo that went out to hospitals and On Line Medical Control physicians about options for administering TXA in the case of saline shortages.
Should you have any questions please reach out to your Regional Office or Maine EMS. We ask that if you find your agency unable to get fluids of any type to alert Maine EMS immediately.
